Camden Minding the Gap Meeting

The Camden Minding the Gap Meeting is a fortnightly meeting for professionals that facilitates young people’s transition from agencies working with young people, (including Child and Adolescent Mental Health Services) into adult mental health services. We also review cases in adult services where there is concern about young people disengaging from services and/or risk in the context of their mental health needs.
Senior clinicians from CAMHS, adult mental health services and other young people’s services in Camden attend the fortnightly meeting to assist referrers in developing a management plan for young people transitioning into adult mental health services.
The case discussion meeting is for professionals only and takes place fortnightly on Thursdays 9.30 – 10.30am on Microsoft Teams. It is open to professionals from all agencies.
The transitions team are a non-case holding team. We will not speak or work directly with the young people discussed at the meeting.
There is a psychologist based within the Personality Disorder Service to champion the cause of young people, help develop a more young-people friendly environment, and assist both with transition from CAMHS and work with young adults coming into services for the first time. Access to Transition Champions will be via the Minding the Gap meeting.

Who is this service for?
The young person must be a Camden resident, between the ages of 17 and 24, and must give their consent to be discussed at the Minding the Gap meeting.
What to expect
Presenting at the Minding the Gap meeting:
We allow twenty minutes per case discussion. This includes ten minutes for the referring clinician to present. It is helpful to include the following:- Current presenting difficulties
- What the young person wants/is looking for/wants to work on
- What are the network’s concerns
- Psychiatric history – diagnosis, previous assessments/interventions & their outcomes
- Risk, substance use, forensic history
- Family, social, support set-up
- Developmental & personal history
- Education, employment, future plans
- There is then ten minutes for all attendees in the meeting to ask questions and discuss which service(s) would be the best options for support for the young person and their family. The discussion is noted and will be shared with you usually the following day.
