
The service is for adults with a history of psychotic disorders, bipolar and complex needs who may have had difficulty working with other teams or services and who may have had many admissions to hospital.
We coordinate care, offer recovery focussed interventions, psychological, practical and emotional support, motivational interviewing, Mental Health Act expertise.
Referrals are accepted via rehabilitation and recovery teams, early interventions service and inpatient wards. We only accept referrals for service users who have an allocated care coordinator.
We provide a team-based approach with individual care coordination. There is a focus on recovery goals, regular medication reviews and help with a range of problems such as substance misuse, housing and benefit issues, emotional well being and extra support in times of crisis. We work with smaller caseloads intensively and can also provide support around physical health needs.
