Islington Community Rehabilitation Team

  • Islington community rehabilitation service provides intensive support and rehabilitation to individuals living in 24 hour supported housing or residential care in Islington who have a primary diagnosis of complex psychosis (this includes schizophrenia, bipolar affective disorder, psychotic depression, delusional disorders and schizoaffective disorder)  to enable them to gain the skills and confidence for successful community living.

    The team includes psychiatrists (doctors), nurses, social workers, a psychologist, an occupational therapist, specialist support workers and clinical researchers. We work closely with the support staff at each accommodation setting in the Borough to help service users reach their individual recovery goals and promote independent life skills. We also have close links with other organisations in the community, including Hillside Clubhouse for employment support, the Recovery College for educational and vocational training, as well as many other services that provide social, occupational and leisure opportunities. 

    We also provide medical and multidisciplinary input into the Islington Community Rehabilitation Units Aberdeen Park and Highview.

How to access this service

Referral is via your care coordinator from your existing mental health team. 

Please send a completed referral form to nlft.islingtoncommunity.rehabteam@nhs.net

Waiting times for assessment and treatment

Your referral will be assessed for eligibility within 4 weeks of receipt. If you are eligible for our service we will arrange a transfer of care from your current team to our team as team capacity allows. We run an active waiting list for our service. 

What to expect

You will be allocated a care coordinator from our team, who could be from a range of health or social care backgrounds but will have experience delivering person-centred rehabilitation for people suffering from long term severe mental health conditions.  

Your care coordinator and psychiatrist will work with you and the team at your accommodation to develop a care plan that meets your individual treatment and recovery goals and helps promote recovery and independence. We will monitor your mental health and support you to access services and resources in the community. We will work with your family carers and offer them support and information.
