Homeless Health Inclusion Team

  • Our team consists of a psychiatrist, mental health nurses, a psychologist, and a dual diagnosis worker, along with dedicated administrative support.

    We work closely with GPs, Haringey Council and StreetLink outreach Team.

    The team provides timely access to advice, assessment and a range of appropriate interventions. They ensure that all service users are treated with dignity and respect without discrimination and according to their needs. The service works together with partners across organisational boundaries to challenge stigma and reduce inequalities so Haringey residents can have the best opportunities and receive the right services at the right time.

    The team’s main function is to reduce health inequalities, by improving the health care and social inclusion of the rough sleeping population.

    The team was previously known as Haringey Rough Sleepers’ Team.

How to access this service

The service will accept self-referrals and referrals from other services i.e. GP, Housing services; Social Care. Referrals will be received by telephone or email and self-referrals will normally be reached through out-reach or the Mulberry Junction, or other venues used by rough sleeping population.

This is checked throughout the day. 
 

What to expect

Our aim is to ensure:

  • People experiencing homelessness receive high quality healthcare.
  • People with a lived experience of homelessness are pro-actively included in patient and public engagement active-ties and supported to join the future healthcare workforce.
  • People experiencing homelessness are never denied access to primary care.
  • Mental Health Care Pathways, including crisis care, offer timely assessment, treatment and continuity of care for people experiencing homelessness.
  • Wherever possible people experiencing homelessness are never discharged from hospital to the street or to unsuitable accommodation.
  • Homeless health advice and signposting is available with-in all urgent and emergency care pathways and settings.
