
DIALOG+ is a patient led outcomes scale whereby people entering secondary mental health services are asked to rate their satisfaction around quality of life and needs for care on 13 areas:
DIALOG+ is a facilitated conversation focusing on the lowest scored areas using the DIALOG scale. The Service User and Health Care Professional work together in a strength-based therapeutic way, co-producing a holistic care plan.
As a minimum, it is recommended that DIALOG+ is completed at the start of treatment, during review dates and routine appointments and at the end of treatment.
