
We provide intensive care and support in patients' homes who may be having a mental health crisis.
We are a team of mental health professionals providing intensive care and support in patients' homes as an alternative to acute inpatient admission.
The service accepts referrals from the community teams, from the primary care mental health teams.
For help from the Crisis Resolution Team, including those who require urgent care should call NHS 111 and select option 2. If you are a patient who needs urgent care you should contact your care co-ordinator first between 9am to 5pm, Monday to Friday. Outside of these hours please call the CRHT team on the number above.
The CRHT service provides an urgent outreach service, and assessment 24 hours a day, seven days a week, wherever people are at the point of referral, e.g. GP surgery, A&E, or in their own homes.
The team will carry out an assessment and wherever appropriate will provide intensive support for a limited period within the service users home. They will work closely with care-coordinators to ensure that care is offered within the least restrictive environment.
If the clinical risks indicate that admission is required then home treatment teams are the gatekeepers of the inpatient beds and will arrange an admission within either one of the assessment wards or in the case of service users well known to services who already have a clear treatment plan, one of the acute wards.
