Health professionals, service users, the voluntary sector and carers can refer to the service.

What to expect

We provide care and support patients through their recovery period and help to get them well enough to be discharged and where necessary receive care in their homes or in the community. 
Our services include:

  • Diagnosis - an appropriate care plan and treatment
  • A physical health check on admission
  • Support to access advocacy services

We also offer support for physical and social needs.

What are the next stages to recovery?

When an inpatient is ready to leave the ward he/she will be supported by the home treatment team for a period of time, who will be able to provide additional support while working alongside the care coordinator.
