Islington Memory Service

  • The Islington Memory and Dementia Navigation Service is an assessment and post-diagnostic support service comprised of Doctors, Nurses, Dementia Navigators, Psychologists and an Occupational Therapist.


    The service aims to support individuals with a diagnosis of dementia to live well in the community.

Conditions treated

Treatment and on-going support are offered to individuals with a diagnosis of Dementia. This may include medication where appropriate, Cognitive Stimulation Therapy (CST), advice and signposting.

How to access this service

Referrals to the service are accepted from GPs, hospitals, and other secondary (specialist) healthcare teams.

Who is this service for?

Assessments

Assessments are offered to Islington residents over that age of 65 where there is a concern the person has a dementia.

Residents under the age of 65 should be referred to the National Hospital for Neurology and Neurosurgery, Queens Square. However, we do offer assessments to residents under the age of 65 who have a severe and enduring mental health problem.

Post diagnostic support

This is offered to all Islington residents with a diagnosis of dementia who live at home.

What to expect

You will receive a phone call to arrange an appointment for an initial assessment, which may be with a doctor or nurse assessor. If you are housebound or there is another reason why you cannot come to our clinic, we will see you at home.

You may be referred for further diagnostic tests such as a CT head scan.

We will arrange a further appointment to feedback your test results and diagnosis. If you receive a diagnosis of dementia, we will discuss a treatment plan, and you will be offered follow up appointments with a named worker every 9 to 12 months. Follow up appointments will be with a Clinical Specialist Nurse or Dementia Navigator.

Through our post-diagnostic support pathway, we connect people to services and signpost to further help as needed. 

Islington has a strong dementia-friendly network and well-established links with local community resources, including Age UK Islington/ Islington Carers Hub and Alzheimer’s Society Islington. 

The Memory and Dementia Navigation Service operates Monday to Friday, 9am to 5pm, excluding public holidays.

If you require urgent mental health support, please contact our older adults Home Treatment Team on 0333 200 7193. You do not need a GP referral to access this service.
