External referrers (GPs, A&E doctors, police doctors and service users or carers) can make a referral via the Barnet Crisis Resolution Home Treatment Team. Care co-coordinators or other internal teams can also refer direct to the home treatment team who then manage all hospital admissions. All referrals will be over seen by Barnet Crisis resolution and Home Treatment Team.
This service is provided for working age adults aged (generally) between 18-65 years, with discretion on people over this age being admitted depending on their individual circumstances who need admission to hospital for assessment and treatment for their illness. People are admitted to this service for assessment and will usually have been diagnosed as having acute and/or enduring mental health problems who cannot be cared for at home or in the community.
The team on Thames Ward provide care, support and treatment in an inpatient setting, to people in an acute phase of mental health needs. We will support and treat people until they are well enough to be discharged from hospital and where necessary, receive care in their homes or in a community setting.
Our vision is to support healthy lives and healthy communities through the provision of excellent integrated mental and community healthcare.
When an patients are ready for discharge this will often be supported by the home treatment team for a period providing additional support whilst working alongside the care coordinator, carers and other stakeholders.
